Liraglutide Peptide is a human glucagon like peptide-1 (GLP-1) receptor agonist. It consists of 31 amino acids and is a linear polypeptide, unique in that a C16 fatty acid side chain is connected to the 26th lysine. This structural modification endows Lilalutide with many excellent properties, making it play an important role in the treatment of diabetes and weight management. There are two key changes compared to natural GLP-1 molecules. One is that the 34th lysine of the natural GLP-1 molecule is replaced by arginine, and the other is that a glutamic acid mediated 16 carbon palmitoyl fatty acid side chain is added to the 26th lysine. This unique molecular change brings multiple advantages.

Liraglutide COA

Liraglutide Peptide, as a long-acting GLP-1 receptor agonist, is currently approved globally and in China as a core hypoglycemic drug with dual indications for blood glucose control and cardiovascular protection. In type 2 diabetes patients with atherosclerotic cardiovascular disease (ASCVD, including coronary heart disease, myocardial infarction, ischemic stroke, peripheral artery disease), heart failure, hypertension, and dyslipidemia, it has multiple clinical values of "reducing blood sugar, weight, heart, and kidney".
Precise target population for combined cardiovascular diseases
Diagnosis of atherosclerotic cardiovascular disease
Coronary heart disease: history of old myocardial infarction, unstable angina, stable angina, and coronary artery revascularization (PCI/CABG).
Cerebrovascular disease: ischemic stroke, transient ischemic attack (TIA), asymptomatic cerebral infarction.
Peripheral vascular disease: Lower limb arterial occlusive disease, carotid artery stenosis ≥ 50%.
Combined heart failure
Heart failure with reduced ejection fraction (HFrEF), heart failure with preserved ejection fraction (HFpEF), NYHA grades II-IV.
Multiple cardiovascular high-risk factors
Age ≥ 55 years old+hypertension+dyslipidemia+smoking+obesity (BMI ≥ 27 kg/m ²), any two or more of them.
Non applicable population (contraindications/caution)
Type 1 diabetes, diabetes ketoacidosis; Personal/family history of medullary thyroid cancer (MTC) and multiple endocrine neoplasia type 2 (MEN-2).
History of recurrence of acute pancreatitis and chronic pancreatitis; Severe liver and kidney dysfunction (eGFR<15 ml/min/1.73m ²) should be used with caution.

The core mechanism of action of liraglutide in cardiovascular protection
Improving endothelial function: Activating GLP-1R in endothelial cells, promoting nitric oxide (NO) synthesis, dilating blood vessels, and improving endothelial dependent vasodilation function (EDV) by 30% to 40%. Inhibit endothelial cell apoptosis, reduce endothelial damage markers (endothelial microbubbles, sICAM-1), and reduce the risk of atherosclerosis initiation.
Inhibit plaque progression and stabilize vulnerable plaques: reduce vascular smooth muscle cell proliferation and migration, inhibit plaque volume growth (animal experiments inhibit 7.8%). Reduce macrophage infiltration, inflammatory factors (TNF - α, IL-6, hs CRP), and tissue protease activity within plaques, increase collagen content, and lower the risk of plaque rupture.
Regulating lipid metabolism: reducing triglycerides (-15%~-20%), small and dense LDL-C (-10%~-15%), mildly increasing HDL-C (+5%~+8%), and reducing lipid deposition.
Mechanisms of myocardial protection and anti heart failure
Improving myocardial energy metabolism: increasing the ratio of creatine phosphate to ATP in the myocardium, enhancing myocardial contraction reserve, improving myocardial perfusion, and increasing the 6-minute walking distance by 33m.
Inhibition of myocardial remodeling and fibrosis: inhibits proliferation of myocardial fibroblasts, reduces collagen deposition, lowers left ventricular mass index, and improves left ventricular diastolic function.
Reduce myocardial ischemia-reperfusion injury: activate the PI3K/Akt pathway, reduce ischemic myocardial cell apoptosis, and shrink myocardial infarction area (animal experiment -28%).
Lowering blood pressure, reducing weight, and optimizing metabolism
Hypotensive effect: Systolic blood pressure decreases by 3-5 mmHg, diastolic blood pressure decreases by 2-3 mmHg, independent of weight loss and reducing vascular burden.
Powerful weight loss: an average weight loss of 4.3kg (LEADER) and a waist circumference of 8.2cm, reducing abdominal obesity and cardiac burden.
Anti inflammatory and antioxidant: reduce hs CRP, IL-6, oxidative stress markers (MDA, ROS), and alleviate chronic inflammation throughout the body.
Renal synergistic protection
Reduce the urinary albumin/creatinine ratio (UACR -25% to -40%), delay eGFR decline, and lower the risk of kidney disease progression and cardiovascular death.

Clinical application by disease type: therapeutic value of combining different cardiovascular diseases
Applicable scenarios: 30 days after acute coronary syndrome (ACS), stable coronary heart disease, PCI/CABG surgery, and recurrent angina pectoris.
Core benefit: Reduced risk of recurrent myocardial infarction and cardiovascular death (HR=0.82). Improve myocardial ischemia, reduce the frequency of angina attacks, and enhance exercise tolerance. Lowering blood sugar without increasing hypoglycemia, without causing heart failure, and without inducing stent thrombosis.
Usage: Starting at 0.6mg qd for 1 week → maintaining at 1.2-1.8mg qd, without the need to adjust dosage due to coronary heart disease.
Applicable scenarios
Previous ischemic stroke TIA, Asymptomatic carotid artery stenosis (≥ 50%).
Core benefits (confirmed by Chinese research)
The recurrence rate of stroke was 7.9% vs 13.8%, with a risk reduction of 44% (HR=0.56).
Improve 90 day neurological outcomes and reduce disability rates.
Reduce the risk of post-stroke depression and cognitive decline.
Combined heart failure (HFrEF/HFpEF)
Applicable scenarios
NYHA grades II-IV, LVEF ≤ 40% (HFrEF) or LVEF ≥ 50% (HFpEF).
Core benefits
Reduce the risk of worsening heart failure hospitalization by 18% to 27%.
Improve 6-minute walking distance and alleviate exertional breathing difficulties.
No heavy water and sodium retention, no negative muscle force effect.
attention
Severe heart failure (NYHA IV) requires monitoring of volume; Used in conjunction with SGLT-2i as a golden combination.

Frequently Asked Questions
Is liraglutide a peptide?
+
-
This medicine is a glucagon-like peptide-1 (GLP-1) receptor agonist. Liraglutide injection is also used together with a reduced-calorie diet and proper exercise to help patients with obesity lose weight. This medicine is available only with your doctor's prescription.
Do you lose weight on liraglutide?
+
-
Liraglutide decreases your appetite. It helps promote weight loss or maintain weight loss. A healthcare provider may recommend changes to your diet and exercise if you take this medication. It comes as an injection that a provider will teach you how to safely administer as directed.
Is liraglutide stronger than Ozempic?
+
-
Ozempic® (semaglutide) typically leads to more weight loss than liraglutide medications like Saxenda® or Victoza®, making it the stronger option for most people. Many people also prefer Ozempic's once-weekly injection schedule over liraglutide's daily dosing.
Who cannot take liraglutide?
+
-
Tell your doctor if you or anyone in your family has or has ever had thyroid cancer or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2; condition that causes tumors in more than one gland in the body). If so, your doctor will probably tell you not to use liraglutide injection.
